Enrollment

All children, regardless of race, color, religion, gender, or national origin, are welcome at Thrive Childcare. To begin the enrollment process, please contact the appropriate center.

The following items are necessary for enrollment:

• Completed registration forms
• Child Enrollment and Health Information form
• Child Medical Statement (to be completed by your doctor)
• Completed ACH (tuition billing) form
• Paid, non-refundable registration fee
• Paid, non-refundable first week’s tuition
• Other documentation as needed (i.e. custody papers, health plans, etc.)
